• REGISTRATION FORM (TO BE COMPLETED BY PARENTS/ GUARDIANS ? 18 YEARS OR OLDER)
• SECURITY CLEARANCE FORMS (TO BE COMPLETED BY PARENTS/GUARDIANS WHO WISH TO COACH/MANAGE A RAIDER TEAM) ? THIS FORM MUST BE WITNESSED AND PROPER IDENTIFICATION MUST BE VIEWED BY  AT EDMONTON WEST RAIDERS OFFICE OR TIME OF REGISTRATION
• ACCEPTANCE/DECLINE POLICY FORM TO BE SIGNED BY THE PARENT AT THE PARENT SIGNATURE LINE AND WITNESSED AND DATED ONLY
• RAIDERS TRYOUT FEE  IS NON REFUNDABLE
• Additional costs apply for Raiders and Warriors program and practice space.
